The CiCi phone card, which I've been using for a couple years now, seems to
be the best one out there, and is significantly cheaper than typical 5 cent
per minute (or greater) long distance plan per-minute rates most companies
offer. Unlike some calling cards there's no connection fees and what not, so
you actually get what you pay for. Once you buy a card (lowest denomination
is $5) it is valid for 6 months from the time it is first used.

2.5 cents a minute to US and Canada.
2 cents a minute to UK.
2.9 cents to India.

Those are just a few of the examples of their low rates.

CiCi phone card has minute-by-minute billing and no extra service or
connection fees. Toll free access number could be used from anywhere in
Canada without extra charge as well. The only extra charge is 50¢ for use
from public phones.

There are calling cards with even lower rates out there, but the quality of
the connection tends to be noticeably poor compared to CiCi, and the lower
rate is made up for by hidden charges, 3 minute minimum increments, quick
card expiry, etc.
