Yes.

I didn't. The device shipped with (iirc) 3.3.6, and the cisco website
had 5.1.10. I have updated the device, though the behaviour persists.

Yes, that is enabled.

Yes, actually. I added a '1', and the CID number comes across properly
now, and the 1 acts is displayed like a country code. I don't know how
international calls would look, but I have never had one before anyway.

For callers without a name (eg. my cell phone only displays a number),
the name "Display Name" on the 3102 is provided. I worked around that by
setting the display name to "Unknown".

I had some weird ring issues (full ring, ½ then ¼ ring, a blip and a
full ring, etc). After setting the answer delay back to 3, it seems to
have gone away. Not sure what that was.

Now to figure out how to disable local voicemail storage, and fix my
dial plan :)
