On my bill there is a line item for "Tax&Others" (sic), with a tiny 
number 4 superscript. At the bottom of the following page are the 
superscript notes, and this:

4. Tax&Others: This includes the tax calculated based on the applicable 
rate for your current services, along with a $2.50 network maintenance fee.
